What exactly am I looking for, if I need a FULLY custom made silver bracelet?

Situation: You know the bracelets alot of us are given the day we are born? Usually says our name, and our birthdate. A very close friend of mine has his, and it's very important to him. This bracelet is of course rather small, with an oval frame peice. There is a alot of detail to it, complete with an attached baby alligator, and of course his birthday is engraved into the bottom side.

Question: I am wondering what exactly I would be looking for to have this bracelet completely replicated. I understand how to find engravers, i'm more concerned about the baby alligator. While I know engraving is when they dig out peices of metal, this alligator is more of a "sculpted" peice, so i'm not sure what this would be called in the custom jewelry making world.

Any of your help is greatly appreciate.
